How are covert missions unlocked?

In Broforce, covert missions seem to randomly appear as the campaign is completed. Some players report seeing 3 or 4 in the first ten levels, others (like me) have completed the campaign with only unlocking two. This is even with all the bros unlocked.
My question is: what are the unlocking criteria for the covert missions in broforce?
